How to Apply

Application Requirements

All items listed below must be completed and submitted at the time of application. Incomplete applications will not be considered.

Eligibility and Education

1. Age Requirement

Applicants must be at least 18 years old.

2. High School Transcript or GED

Submit an official high school transcript showing a minimum 2.0 GPA or official GED documentation.

International Students: Students who completed coursework outside the United States must have their transcript(s) evaluated for U.S. equivalency by:

Health and Clinical Requirements

3. Physical Health Evaluation

A completed Physical Health Evaluation must be submitted with your application.

The required form is provided by the school.

Ensure your health care provider signs and stamps the bottom of the form. If a stamp is not available, a photo of the provider’s business card may be submitted instead.

The physical may be completed no earlier than six months before the class start date and must be completed by the time you submit your application.

Example: If the class begins August 20, the physical must be completed between February 20 and the date you submit your application.

4. TB Clearance

TB clearance must be submitted with your application.

Submit documentation of one of the following:

• Negative TB skin test
• Negative QuantiFERON blood test
• Negative Chest X-Ray with Chest X-Ray Questionnaire

All of the listed items may be completed no earlier than six months before the class start date and must be completed by the time you submit your application.

Example: If the class begins August 20, the test or chest x-ray must be completed between February 20 and the date you submit your application.

5. Current CPR Certification

Submit a current CPR card or certificate that includes the American Heart Association (AHA) name or logo.

Your CPR certification must remain valid and must not expire at any time during the program.

6. Tetanus Shot (Tdap)

Submit documentation of a current Tdap vaccination.

Tdap vaccinations are valid for 10 years. Your Tdap must remain valid (not expire) at any time during the program.

7. Hepatitis B Requirement

Applicants must meet one of the following requirements at the time of application:

Option A: Hepatitis B Vaccination

Submit documentation showing that you have:

• Completed the three-dose Hepatitis B vaccine series;
• Completed the two-dose Heplisav-B vaccine series; or
• Received at least the first dose of a Hepatitis B vaccine series.

If your vaccine series is not complete at the time of application, documentation of each additional dose must be submitted as it becomes due. The full series must be completed while you are enrolled in the program.

Option B: Proof of Immunity

Submit documentation of a positive Hepatitis B titer.

Requirements After Conditional Acceptance

A background check and drug screening will be required after conditional acceptance into the program. Selected students will receive additional instructions during the mandatory orientation.
