- If the university permits the student to use the electronic version of the material outside of direct staff supervision, the disk or file shall be copy-protected or
the college or university shall take other reasonable precautions to ensure that the student does not copy or distribute electronic versions of
instructional materials.
To save you time, this is the statement from you that publisher John Wiley &
Sons will request when you want e-text.

1. The Electronic Copy is for use by a student who is registered at the university who has a disability that

The form the publisher has online is not for electronic forms of texts. If you need the e-text of one of their books, you must
contact the acquisitions editor for that particular book. I called the toll-free number and asked for customer service who, in
turn, gave me the name, email address, and extension of the editor I needed to contact. I have not had a response yet, since I
made the inquiry today (Nov. 19, 2002), but I will keep you all updated.

- "I have purchased a copy of ABC book from the [College Name] bookstore at the cost of $$" (signed by the student) It is necessary to provide copies of
bookstore receipts to this publisher, or you will be billed.
