Nurse Assistant Program

 

Admission Procedures

 

 

 

Orientation:  Students must attend a Nurse Assistant Orientation . Students must purchase a Merced College Nurse Assistant Program Handbook at the Merced College bookstore prior to orientation. The orientation will provide the necessary information needed to successfully complete the requirements prior to registration.  The information listed below will be need to be submitted to the Allied Health Office:
1.

Clearance of Conviction

The prospective student must sign the Clearance of Conviction form (located in handbook) stating they have not been convicted of any crime other than a minor traffic violation. If you have a violation, please contact the Department of Public Health at 916-327-2445 to request a clearance of conviction. Nurse Assistant Certification is prohibited if there are any convictions listed unless the action has been dismissed or the individual has obtained a Certificate of Rehabilitation. The clearance issued from the state must be submitted to the Allied Health secretary prior to registration.

Nurse Assistant Initial Application (HS 283 B forms)

Two applications for Nurse Assistant (located in the handbook) must be completed.

Live Scan Fingerprinting

It is required of all Skilled Nursing facilities to fingerprint all Nurse Assistant students and prospective employees. This is affected by the Health and Safety Code 133.8 and 1736.1. Effective May 18, 1999, prospective students will be Live Scan fingerprinted. This fee will be paid for by the training facility not the student.

Physical Exam and Health Questionnaire

Students must fill out the Health Questionnaire and have a Physical Exam no later than 6 months prior to enrollment.

TB skin test (Purified Protein Derivative/Tuberculosis)

Students must have a PPD skin test. If previous test was positive, submit proof of a negative chest x-ray.

CPR

Current CPR card either the Health Care Provider or The Professional Rescuer (that does not expire during semester you are in NA program) is required prior to registration.

7.

Registration

After the student has completed ALL of the requirements listed, he/she will then submit all paperwork to the Allied Health Office (V-116). After the Allied Health Office has obtained Certification Abuse Registry Clearance, the student will then be given a voucher to register for class.

 

Upon successful completion of the Nurse Assistant Program, the student must complete the Certification Exam in order to become a Certified Nursing Assistant. The exam has been developed to meet the evaluation requirement of federal and state nurse aid competency evaluation legislation. The test is offered monthly throughout the state. The test may be offered at Merced College upon completion of each training course. The tests consists of 2 parts: written and manual. The fee is $90.

NEW STUDENTS WILL BE REQUIRED TO APPLY FOR COLLEGE ADMISSION AND COMPLETE APPROPRIATE REGISTRATION PROCEDURES, AND OBTAIN A STUDENT I.D. CARD.
