A student-athlete may not compete in an intercollegiate sport until first completing a physical examination from a physician (M.D.). Fill out the following document and have a physician review it with you and sign it. Then return it or fax it to the athletic training facility at Merced College. A coach or athletic trainer may ask you to fill out a medical history questionaire before taking your "physical". This form does not replace the "physical".
